Based on the teachings in this chapter, why is the constant remembrance of God's name considered so important, especially in relation to one's final moments?

📖 Chapter 31

Chapter 31 explains a profound spiritual principle: one's state of mind at the moment of death determines the state the soul attains afterward. It states that whatever one meditates upon at the time of death, that very form one attains in rebirth. The practice of remembrance of the Name is emphasized so that one does not get confused during difficult times and this remembrance remains firm at the very end. The text warns that even a lifetime of spiritual wakefulness can be rendered in vain if one "fell asleep" at the end, making constant practice and satsang essential.
